Your DOT medical certificate is one of the most important documents you carry as a commercial driver. Let it expire and you're off the road — legally and immediately. Here's everything you need to know about validity periods, renewal timing, and what to do if yours has already lapsed.


How Long Is a DOT Physical Valid?

A DOT medical certificate is valid for up to 24 months for drivers in good health. However, if your examiner identifies a managed condition, your certificate may be issued for a shorter cycle:

  • 12 months — common for drivers with blood pressure in Stage 1 range or other monitored conditions

  • 6 months — sometimes required for drivers with more closely managed health factors

  • 3 months — issued in specific cases requiring frequent reassessment

A shorter cycle doesn't mean you can't drive — it simply means you'll need to renew more frequently to confirm your condition remains stable.


When Should You Renew?

Renew at least 30 days before your certificate expires. This gives you a buffer in case any health issues need to be addressed before certification is issued.

Set a reminder the day you receive your certificate. Two years passes faster than most drivers expect.


What Happens If Your DOT Physical Expires?

The consequences are immediate:

  • You are not legally permitted to operate a commercial vehicle

  • Your employer is required to remove you from driving duty

  • Your CDL status may be affected depending on your state

  • Lost driving days mean lost income

There is no grace period. If your certificate has already expired, schedule your exam as soon as possible — many drivers are back on the road the same day.

  • Valid driver's license
