DOT Physical Blood Pressure Requirements: What CDL Drivers Need to Know

One of the most important parts of your DOT physical exam is the blood pressure check. Your blood pressure reading directly affects how long your DOT medical certificate will be valid — so understanding the guidelines can help you prepare and know what to expect.

FMCSA Blood Pressure Guidelines

The Federal Motor Carrier Safety Administration (FMCSA) uses blood pressure readings and documented history to determine the appropriate certification period. In accordance with 49 CFR §391.41 and FMCSA hypertension guidance, drivers whose blood pressure falls within this range may still be certified; however, the medical examiner may issue a certificate for a reduced duration based on the hypertension stage and the driver’s documented history

Normal to Stage 1 (below 140/90):Drivers with blood pressure under 140/90 typically qualify for the standard 2-year certification, provided all other health measures are within normal limits.

Stage 1 Hypertension (140-159 / 90/99):If your blood pressure falls within this range, the medical examiner may issue a medical certificate; however, consistent with 49 CFR §391.41 and FMCSA hypertension guidance, the certificate may be limited to a duration of less than one year based on the hypertension stage and your documented history.

Stage 2 Hypertension (160-179 / 100/109):

Drivers in this range may receive a temporary certification of up to 3 months, allowing time to bring blood pressure under control before recertification.

Stage 3 / Hypertensive Crisis (180/110 and above):Drivers at this level are not eligible for certification until blood pressure is brought under control.

Why This Matters for CDL Drivers

A shorter certification period means more frequent visits and paperwork — something every commercial driver wants to avoid. If you know you have elevated blood pressure, here are a few tips before your exam:

  • Take prescribed blood pressure medication as directed

  • Avoid caffeine and heavy exercise right before your appointment

  • Sit quietly for a few minutes before your reading is taken

  • Bring a list of current medications to your exam
